Hours: Monday: 10-2 & 3:00-7:00 Tuesday, Wednesday, Thursday, Friday: 8:00-12:00 & 1:00-5:00 Open Fridays thru June 2014 8:00-5:00; closed on Fridays effective July 2014. - Postpartum non-breast feeding up to 6 months after delivery - Breast feeding up to 1 year - Infants & children under 5 - Income at or below 185% of federal poverty guidelines Intake procedure:- Appointment required - Health and nutrition risk screening - Must pick up and use all WIC checks issued - Must participate in nutrition education and/or counseling What to bring: - Proof of income or receipt of public assistance - Proof of identity - Proof of residence in the State of Indiana
